*Please select up to 4 subjects from Blocks A-E (together with a subject from the Additional Subjects Block if you wish) which you would like to study. If you chose Maths in Block A and Further Maths in Block C (i.e. not in the Additional Subjects block) you may chose a subject from each of Blocks B,D and E, giving a total of 5 main subjects. However, in this case you may not chose a subject from the Additional Subjects Block.

**Further Maths may only be chosen once (i.e. either from Block C or the Additional Subjects Block) and must be in addition to Maths in Block A.
